CDC Nominations for Promising Interventions to Control High Blood Pressure
ICF International is soliciting nominations for innovative programs and policies with the potential to address high blood pressure at the population level. We are interested in policy approaches and system-level strategies in two distinct areas:
1) Policy and systems strategies that promote the use of Patient-Centered Medical Homes, which provide quality-driven, cost-effective, and culturally appropriate coordinated care
OR
2) Policy and systems strategies used by employers that support the control of high blood pressure excluding worksite wellness initiatives. For example, strategies like value-based insurance designs and other essential health benefits

Key Activities for Participating Programs:
- Nominate a program or policy
- Nomination review by a panel of experts
- If selected by the panel
- participate in a 2 1/2 day evaluability assessment site visit to explore program implementation, data collection, and program outcomes
- possibly be featured on the Web
- receive on-site technical assistance and follow-up consultation
- If selected as a potential promising intervention
- participate in a comprehensive evaluation
